Social Services acts as part of a team and provides services to renal patients. The social workers focus on assessing needs, setting up community resources and support for the patient and caregivers, and working as a liaison for patients.
The social worker also travels to our satellite clinics coordinating services for these patients and families with use of local resources and services where able.
